Summer Fun Day Camp 2011

7 Week Program—June 6 to July 22
*** No camp Monday, July 4 ***

Monday thru Friday
7:30am to 5:30pm

Ages 5 to 12
*** Children MUST be 6 years old by September 30, 2011 ***
*** Children MUST NOT be 13 years old before June 6, 2011 ***

SUMMER CAMP PROGRAM EVALUATION

Parents,
We hope that your child had a wonderful experience participating in this year's Summer Camp program. In order for us to evaluate the operation of this year's program and to better prepare ourselves in organizing next summer's program, we could greatly use your feedback and input. Please print the Parent Evaluation Form, fill it out and mail it back in to us. Your comments are valuable to us and will help us to make next year's program even bigger and better.

Registration Fee: $25.00 per child

Camp Fees: $17.00 per day, per child

Financial Assistance is available.

Children need to bring a packed lunch with a drink & a snack everyday.
